The NHS has agreed a service to help people with complex physical or mental health needs to keep well and avoid unplanned hospital visits, such as in an emergency. Westside Surgery will identify these people and invite them to receive a more tailored, active support from the practice.


Patients will have a named GP who will have overall responsibility for their care and support and a named care co-ordinator, who may be the named GP or another health professional involved in their care.

These new arrangements do not prevent patients making an appointment or seeing any doctor, of choice within the surgery, as normal.

The named GP or named care co-ordinator will work with the patient to develop a personal care plan and review / discuss with them any changes needed. We will also make sure that, whenever the patients has an urgent need to see or speak to a GP or nurse, as soon as possible on the same day one of our GPs or nurses will phone them back to help deal with the problem.
